The first six weeks decide a lot
Rehabilitation after surgery has a window. Scar tissue matures, joints stiffen in the position they are held in, and muscle wastes quickly around an operated limb. What happens in the first six weeks sets a ceiling that is difficult to raise afterwards: particularly for knees, which stiffen readily and are stubborn to free again.
This is the phase when getting to a clinic is hardest. You cannot drive, sitting in traffic is genuinely painful, and every appointment is an ordeal. That combination is why so many people fall behind their protocol in exactly the weeks that matter most, and it is why we run this service as a home visit by default.
Following your surgeon’s protocol
Bring whatever your surgeon gave you. Protocols vary between surgeons and between techniques, and the restrictions are not arbitrary. They reflect what the repair can safely take at each stage. We work within them and contact the surgeon’s rooms if anything needs clarification.
The phases
- Protect and settle: swelling control, gentle range, safe walking
- Restore movement, full range before we chase strength
- Rebuild strength: progressive loading, both sides, not just the operated one
- Return to function: stairs, driving, work, and sport where that applies
Skipping ahead is the most common self-inflicted setback we see. So is stopping at phase three because the pain has gone, which is how people end up with a knee that works fine until the first time they need it to do something demanding.

Common questions
When should rehabilitation start after surgery?
Usually within days, and for joint replacements often before you leave hospital. The early phase is about swelling, gentle range of movement and safe walking, not strength. Delay is the most common cause of a stiff result.
Can you come to my home while I cannot drive?
Yes, and this is exactly when home visits matter most. The first few weeks after surgery are when travel is hardest and when consistent rehabilitation makes the biggest difference.
How long until I am back to normal?
For a knee replacement, expect three to six months for comfortable daily function and up to a year for the final result. ACL reconstruction is typically nine to twelve months before a return to pivoting sport. We will give you the timeline for your specific operation.
What if I am behind where the protocol says I should be?
Common, and usually fixable. We reassess, work out what is limiting you (swelling, pain, fear of loading) and address that rather than pushing blindly against it.
